ACTUAL BEHAVIOR
---------------
Cannot create Opportunity from an Account

When clicking on the 'Create Opportunity' from an Account, the system opens a blank screen and the user has to exit the application.
The issue is happening for all users.

Users are able to create an Opportunity from the Opportunity tab, but most all users create from the Account page.

EXPECTED BEHAVIOR
-----------------------
Should be able to create Opportunity from Account tab.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Account
2. Go to Opportunity subtab to create an Opportunity from an Account
Observe that a blank screen is shown and user has to exit the application.
