Symptoms

All Transactions are appearing blank and failing. Some examples are the New Hire and Document of Record approvals. But confirmed all notifications are affected.
When the transactions are submitted, the bell notification is reviewed and it is blank with showing minimal information at the top of the notification (i.e. name of transaction and employee name)
