"Invalid User Name or Password" Exception when Accessing OTM as a Newly Created Service Provider User

Symptoms
When accessing OTM as a newly created service provider user, the user fails to login to the instance with the following exception observed at the login screen:
STEPS
--------------------
1. From Configuration and Administration -> User Management -> User Manager create a new user in the SERVPROV domain.
2. Assign that user the SERVPROV role and password.
3. Click Finished to created the user
4. Attempt to login with the newly created user and the exception will be observed.
