Symptoms

Extra fields are displayed on Case Form >> General tab which exists duplicately on other Case Form tabs.
-- 患者に関する記述情報 (Same as Case Form / Patient / Notes)
-- 臨床経過及びその他の記述情報 (Same as Case Form / Analysis / Case Analysis / Narrative)
-- 報告者の意見 (Same as Case Form / Analysis / Case Analysis / Case Comment)

This symptom is not observed on English language mode Case Form but is observed on Japanese language mode Case Form.
