Symptoms

Cloud-hosted customer has upgraded 1 environment (such as Stage) to a new major release, while their other environment(s) remain at a different major release.

When attempting to access the upgraded environment, users are re-directed to the other environment and are unable to log into the upgraded environment.

For example:

Customer's Production environment is release 16 or 17, and their Stage environment has been upgraded to release 18.
