[ACCS] Deploy Application Fails with Error "Application connection failure. Unable to connect to application at root URL." (Doc ID 2513099.1)

In Application Container Cloud Service (ACCS), attempting to deploy a new release of an application sometimes fails. This can also occur with application start-up failures.
The Activity Log shows the error:

The application becomes unavailable with a red triangle over its symbol.

Deleting the application and creating a new one with the same name, then deploying the same archive (that was failing) again sometimes works to get past the problem, at least for some time.  However, this does not always work.
