Symptoms
Input and Output Interfaces
When trying to upload data at screen: Input Interface receive error.
This error only occurs for one facility and input type Location
Same facility and different input type such as Purchase Order do not error.
Other facilities and all input types such as,Location do not error.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Input Interface
2. In drop drop, select "location" save,
3. Receive error
Unable to load ../service_gateway/?
request.preventCache=1554760345925 status: 500
(see cursor spinning)
