CSV Import: "Records creation in progress..." Creation Fails When There Is More Than One Data Element With The Same Label

When a CSV file Template is imported to create record and/or line item data:
After the CSV file is imported and the user is notified that 'Records creation in progress. You will be notified by email when process is complete' the record(s) should be created and found in the applicable workspace.
A Failed e-mail notification is received indicating the Reason is that some value needs to be added. E.g., Result: Failed, Reason: <Name>: Please add <name> on detail line item
The Reason can also indicate the <name> needs to be added to the upper form.
When checking the CSV file the <name> is populated.
Due to this issue, users cannot create the record from the csv import.
