Symptoms

On : 11.13.18.10.0 version, Service Requests

ACTUAL BEHAVIOR
When selecting Actions> Resolve... the Resolve Service Request popup page shows a blank screen.

EXPECTED BEHAVIOR
The screen should not be blank and show the fields on the page

The issue can be produced with the the following steps:
1. Edit an SR
2. Click on the Actions menu and select Resolve option
