Blank Frame or Error [Refused to connect] when Creating a Quote from Salesforce

Information in this document applies to any platform.
When a user without a token attempts to create a quote from Salesforce, a token should be created if they have correct partner login information in CPQ. Instead, depending on the browser, the user sees either a blank frame or a gray frame with this error:
xxxxx.my.salesforce.com refused to connect
In the Developer Tools console, an error similar to the one below shows:
Load denied by X-Frame-Options: https://xxxxxx.xxxx.force.com/setup/secur/RemoteAccessAuthorizationPage.apexp?source=... does not permit framing.
This occurs only when CPQ loads in an iframe.
The Managed Package or Lightning Connector was recently installed and set up.
