Symptoms

DESCRIPTION:

Error message when trying to add Issues in Commissioning.

Error message pops up as soon as New Issue is clicked within the Commissioning form.

ERROR:

Error Occurred! We apologize for Inconvenience!

STEPS:

The issue can be reproduced at will with the following steps:
1. Go to the Commission Form for a Project.
2. Try to select the New option.
3. Encounter error.
