Approval Rules For Approve Performance Document Not Working (Doc ID 2550652.1)

Client tested the approval rules and the workflow worked correctly for performance documents that contain the text "Coaching" or Corrective". This version of Test was copied from production the week before.

Client tested this workflow again in preparation for actual go live and it did not work as expected.
Client tested the same in other instance last week as it was a recent copy of production with same results.
The employee relations consultant did not receive notification of a performance document to approve; rather the employee received 2 notifications: 1 - created and 2 - evaluated.
