Error While Creating Service Request Categories (Doc ID 2553324.1)

Symptoms

Error when we try to add more children to a top level category. I can have 1 child to the top level but when I try and add more it errors out.

Error:
Attribute CategoryName in SrCategoryTranslationEO is required.
Attribute CategoryName in ServiceRequestAM.SrCategoryParent is required.
Attribute CategoryShortName in ServiceRequestAM.SrCategoryParent is required.
