On any Non-Production instance of Unifier : Any Version

When trying to create a new workflow type Business Process record the following Alert pops up:

The system could not process you rrequest. Logging in again may help in some cases. Contact your System Administrator if you are still having problems.

When the Unifier log files are pulled the following can be found therein:

Can not find upper form prefix and form_id
