Fusion Global HR Submitted' Notification Received multiple times (Doc ID 2571179.1)

Symptoms

In our workflows like  Promote Change manager  etc. the initiator of the transaction is getting 'Submitted' notification after submitting the transaction and when the final approver has approved the transaction.
After the final approval, the initiator should get an 'Approved' notification, which has been the case till now. But recently, we have observed that sometimes, the initiator is getting an 'Approved' as well as a 'Submitted' notification but sometimes, they are only getting either a 'Submitted' or 'Approved' notification.But this notification is very inconsistent. The initiator is not always getting the notification with header 'Submitted' after submitting or after the final approver. We want to understand why and when a 'Submitted' notification is triggered and why is this inconsistent.
