OIC Monitor pages not showing instances in Error state as expected (Doc ID 2585487.1)

The OIC Track Instances and Monitor Integrations pages are not showing errored instances of certain Integrations

But when examining the Integration flow of the instance (eg by clicking on the Start time in the Track Instances page), it can be seen that the an activity has failed, and there was an error at runtime.

Why are the Monitor pages not reflecting this ?
