Cloud User Is Unable To Change Or Reset Password

Information in this document applies to any platform.
A user is unable to "change his password" - gets error IAM-3040003 : An exception was encountered while performing operation.
The cloud admins have reset the password multiple times, and the user account is NOT locked or disabled.
User is able to login using the temporary password (supplied by the cloud admin), but is unable to update his password or complete the challenge questions.
