Footnotes not working for Workforce Comp Change Statements

I am using an RTF format for my change statement template. The change statements configuration to "allow managers to combine multiple statements into one file" is checked. But when I do generate statements for "Selected Workers" the footnotes do not work. I have tried various combinations. All are attached along with output of each and a word doc with screen shots of the configuration. Why do footnotes not seem to follow any BI publisher commands?
Ver1 - set the Word footnote design to have the "different first page" checked; Result: only first employee has the footnote.
Ver2 - Word footnote design does not have the "different first page" checked; result; all 3 employees have the footnote for employee 1.
Ver 3 - added the "For-Each" commands in the footnote section as well; result: first page has 1st employee in footnote; 2nd page has 2nd employee but all 3 employees footnote data; 3rd page has 3rd employee data but footnote has all 3 employee footnote data.

Bottom line: the templates attached are simplified to show the issue with the footnote. Why does any logic in the footnote not seem to be executed? And why is it not kept with the page of data for the employee - we need the body and the footnote to be for each individual employee. We need certain footnotes to be shown on the employee page of data (body) based on if they meet the criteria to show the footnote. Criteria is based on certain fields in the WFC plan for the employee.
