AP Invoice Hold Approval (FinApHoldApproval) Workflow Does Not Trigger Notifications

AP Payables Invoice Hold Approval BPM process does not seem to initiate any notifications. The BPM process is defined to send notifications to the requestor or requestor hierarchy. However, no notifications seem to have gone out.
