Unable to Create Change Order

When the user tries to create change order through UI, clicking on task 'Create change order' refreshes the page and does not let user create the Change Order.
STEPS PERFORMED TO REPRODUCE THE ISSUE
1) Go to Product Management
2) Click on Product Development
3) Click Create Change Order
4) Enter Change Order type
5) Click Save and Edit
