A Questionnaire Template was Created, but cannot be Found in the Manage Checklist Template. (Doc ID 2641103.1)

There is an issue with a questionnaire.

The question template was created with subscriber "HR Checklist" and now we are trying to create a checklist's task with action type "Questionnaire".

However, the template cannot be found in the Manage Checklist template.
Why is the template not displaying?
