Symptoms

 11.13.19.10.0 version
Widespread Transaction Failures due to Health of Managed Servers
Several different transactions across different processes and users are failing. Most of the failures are due to health of the managed servers.When this happens,  we have to terminate the process and ask the user to resubmit which is causing widespread frustration.  We are asking why this keeps happening?  Does Oracle need to reset something on the back-end or find the wayward server?

Fault Code SERVER_AUTO_RECOVER_EXHAUSTED
Issue Type BPEL_FAULT
Issue
Description Potential recoverable issue caused due to health of managed servers.

Resolution:
Verify that all managed servers are up and running and are in a healthy state. Then attempt
recovery. If managed servers are in a healthy state and the issue persists, contact Oracle
Support with the detailed fault message.
