Error When Adding Attachments On All Objects (Doc ID 2660902.1)

Symptoms

When attempting to attach a file to opportunity the following error occurs.

ERROR
-----------------------
Connection to the server has failed (status=unknown)


STEPS
-----------------------

1. Log into application
2. Navigator>Sales>Opportunity
3. select a record
4. click on record name link to drill down
5. Edit Opportunity page opens up
6. go to Attachments > click on add and try to attach a file
7. There is an error displayed

 "Connection to the server has failed (status=unknown)"
