OCI - System don't finish the boot and stay in iSCSI erro loop (Doc ID 2670724.1)

Symptoms

System don't complete the boot and as consequence stay inacessible.

In the console the system is in loop with the following errors:

Changes

Due to a crash a filesystem is corrupted.

To try the boot the iscsi disks where detached and re-attached.
