ERROR: Form Submit App - "400 - BadRequest". (Doc ID 2673350.1)

Symptoms

Contacts are getting stuck in a Form Submit campaign step with the status "Status 'Error' set by an external service". When viewing the Cloud Action Configuration Contact Process Status  errors, a "400 - BadRequest" error is listed.

Reinstalling the app and reflowing contacts does not resolve the issue.

There is no validation on any of the form fields, and test submissions of the form outside of the app are successful.
