Unable To Provision Users After an Environment's Data Has Been Refreshed

When attempting to provision users in Cloud Admin, the Administrator receives a failed message. Cloud Admin log shows the following error.
Creating/Assigning Application Access for Users:
Unassigning Application Access failed : due to Error while deleting user
2020-05-29T12:54:45.746+0000 Revoking Application Access failed for user [USERNAME]
2020-05-29T12:54:45.748+0000 Completed unassigning of Application Access 
2020-05-29T12:54:45.750+0000 Assigned Application Access for User: [USERNAME]
