Oracle Fusion Recruiting Cloud: Extend Offer Not Available in Actions

For two candidates, their offer is approved but there is no option in the Actions menu to Extend Offer -- only Accept Offer.
One of the candidates was rejected by the employer, then a new offer letter was created, edited, submitted, then approved. Still, the approved offer shows no option to extend.
