Oracle Fusion Learning Cloud: Errors Displayed and An Assessment Will Not Complete Even When Passed (Doc ID 2714603.1)

A Learner finishes an assessment with the correct answers and tries to submit but receives an error message saying, "Warning. You didn't achieve a passing score on this assessment. However, you have 998 more attempts to complete this assessment." After clicking OK on the pop-up, a further error is displayed, 'You didn't receive a passing score on the assessment. You have reached the maximum amount of attempts to complete this assessment."  Despite having the correct answers, the activity does not result in a "Completed" status.
