Error Email Message Received From Email Approval Even Though Approval Was Successful

A user that was able to successfully approve a Business Process (BP) record via email; however, the user still received an error email message that it failed.
In the Error email, the workflow starts with the letters "RE". The reason of failure shows "Invalid workflow action name".
There should not be any error email sent from Unifier as the record had been successfully approved via email.
