Action By Email Intermittently Not Updating All Tasks And Records

When using Approval or Action by Task Email on some Business Process records it has been observed that some BP records are not being updated as expected.
Expecting that actions taken via email will be processed consistently and not take excessive time if there isn't a load on the system.
The issue can be reproduced at will with the following steps:
- User receives an Action Email.
- User clicks on "Approve" button in email for example.
- The BP record is not updated to indicate Approval by the Assigned user.
