Symptoms

When attempting to click an action that uses an Integration XSL, an error occurs:

Could not perform Action: Save
During commerce integration Process ID: 1234567, Name: ORCL SFDC Quote - Upsert
An error occurred while communicating with the partner site.
Error message from the partner site:
A duplicate value was specified for field 'cafsl__Account__c' in object 'Oracle_Quote__c', duplicate value 'ABCD' prior value 'EFGH'
