“Generate Document” OTM Standard Shipment Action With "Add New Revision" Checked Is Not Keeping The Old Revision (Doc ID 2761174.1)

A New Document Type is configured to generate a report
The Generate Document action is performed on Shipment. This adds a record under ‘Documents’ tab.
Now again when same action is performed with same document type (but by checking “Add New Revision” check box), Application is replacing existing Revision instead of generating new version

Expectation: When selecting "Add New Revision" check box with generate document action, both old and new document records should be displayed under shipment>>document section

The issue can be reproduced at will with the following steps:
1. Select Shipment >>Actions>>Generate Document once and then a second time with "Add New Revision" check box selected
2. Shipment>>Document tab shows only 1 latest record and not the old record
