Assigning Data Access to Users Using a Spreadsheet error (Doc ID 2768801.1)

Symptoms

On : 11.13.21.01.0 version, Setup, Rapid Impl and Security

When attempting to upload Data Access for users through the security Data Access Template(ADFDI), every time when user insets a record it says insert failed/update failed or record already exists even though none of these exist in the environment.

ERROR

'Insert failed, A record with this combination of values already exists.'


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigator
2. Manage Data Access for Users
3. Download the template
4. Populate the template
5. Upload

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot update or upload data access through Security Data Access Template.
