New Matrix Class is not setting new resolution milestone on SR

I have created two resolution milestones for AT&T
1. AP Helpdesk - based on condition that Severity = High. This one is working as expected.
2. Manager Resolution - based on the condition that Severity = Low. This one is not working and not applying the milestone to the service request at all.
Attached is a document with screen captures and notes on configurations completed in Subscription Entitlement and Milestone and steps for validation.
