Symptoms

When a Learner puts in a Non-Catalog Learning Request, no approval notification is being sent to the Learning Specialist.
When checking the Transaction logs for this approval that failed, the following information is in the Issue Description:

Error caused by SOA component or the underlying HWF configuration or a Supervisor hierarchy setup issue.
