Intermittent: Previously Entered Comment's Date/Time and Text Entry Overwritten by A Newly Added Comment

Symptoms
On Unifier : Version 20.12.3
When adding a Comment to a Business Process record where another Comment has been previously added:
Expected Result
The new Comment should be added without incident and the record should be showing the old item(s) and newly added item.
Actual Result
Intermittently, when one user adds a Comment to a Business Process (BP) record the date/time stamp and text entry will overwrite the data previously added.
Business Impact
Because of this previously existing data is being overwritten and lost.
