SCM: PD: Getting Error When Trying to Approve an Old STALE Change Order

In this note we explore when user is getting an error when tries to open a affected item tab in Change order
Error : Too many matching records found .Specify additional criteria to limit the number of records .
At the same time, when users opens the Workflow it shows it's awaiting approval. Upon Trying to Approve the CO, it receives the following error:
"Task with task id xxxxxxxxxxxxxxxxx is STALE. Trying to access details of stale task. An instance of the application might have been undeployed/ the application might have been redeployed and this task belong to previous deployment. This cannot be corrected. Attempt to perform actions on stale tasks is not permitted."
