Symptoms
Error saving Saved Query definition due to duplicate fields.
Example

User Query Name: XXX
Object Type ID: TRANSMISSION
Use In Finder: Y
Go to the Saved Query Definition tab
Column: Insert Time
Note: You will see two duplicate Insert Time field.
Enter a date in the first filed and click save -> You will get this message
'Insert Time is required'
Enter a date in the second field -> This error happens.
