New Supplier Sites Allows Multiple Primary Addresses

When creating a supplier site address, there is a prompt with the message 'Do you want the primary address for the supplier to default to the address for the supplier site?'. Users are prompted with this every time they add an address when initially creating the supplier site, and if they click yes to the prompt every time, multiple addresses are created and marked as primary. This prompt should only pop up for the first address created.

Steps To Recreate:

1) Create new supplier site
2) Click More Actions button and select address
3) Prompt will ask to copy primary address from supplier and make default address for supplier site, select Yes
4) Repeat steps 2 & 3. Notice how it will allow user to create multiple addresses and they will all be primary
