Onboarding PDF Form Pages Do Not Render Correctly

Symptoms
When editing an onboarding PDF form (or viewing/eSigning a form on the Talent Center), one or more pages on the form do not render. The affected pages include a broken image icon.
The issue can be observed by taking the following steps:
1. Select the Administration menu. 2. Select the Customize Onboard/Offboard option. 3. Under the Customize Forms section, click the Configurable Forms link. 4. Under the Employee PDF Forms section, locate the row listing the affected PDF form. 5. Select the Action menu (next to the View option), and select the Edit option. 6. Review the pane on the left-hand side of the screen for page previews:
