Symptoms

When attempting to create a document, the following error occurs.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) Go to Product Management > Product Development > Manage Documents
2) Click on the + sign and enter the details into the popup
3) See error: An application error occurred. See the incident log for more information.
