What to check if you have Touchless Purchase Order Creation and Requisition lines are not being grouped, are not being grouped properly?

For instance,

Requisition is created with 3 lines for the same item, but different Deliver-to locations
line 1, Deliver-to Location B
line 2, Deliver-to Location H
line 3 , Deliver-to Location V

The  system creates 3 purchase orders after requisition approval
 requisition number = 11
line 1, purchase order 21, line 1, schedule 1, Location B , organization 31
line 2, purchase order 22, line 1, schedule 1 , Location H , organization 31
line 3, purchase order 23 , line 1, schedule 1, Location V, organization 41
