Symptoms

On : 11.13.22.01.0 version, Accounts, Contacts, Households

When attempting to add Assessment under Account, the following error occurs.

For input string: "1 T3 2021"


STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. Edit any existing Account
2. Click on sub tab > Assessments
3. Create assessment with template "Customer Relationship Status"
