Symptoms
When submitting a record to review, the following error is generated:
ERROR
Error Code: [SHORT_NAME]-[ALPHA_NUMERIC_STRING]
An unexpected error has occurred, and the system cannot process your request.
Contact your System Administrator referencing this error code [SHORT_NAME]-[ALPHA_NUMERIC_STRING].
STEPS

The issue can be reproduced at will with the following steps:
- Log into the Unifier application
- Navigate to the affected Business Process (BP)
- Create a new record
- Add the mandatory fields
- Submit the record for review
- Notice the error
