Unable to Start Service Due to Enable Service Activity Error

When the CSR moves in a customer using Start/Stop, the system creates an Enable Service order.
The Enable Service is expected to check the service point and see if there's a meter installed at the SP. If there's no meter installed, system will automatically create an Install Meter FA that will go to OFSC.
Today, we got 130+ D1-D1-ATVTD To Dos - where the error is "Activity Type Field Task Type should not be empty when Service Point Device Condition Source Flag on Activity Type is Table.
What the system is doing is it is creating the correct activity type which is Enable Service, but it is not doing the following actions after the Enable Service:
- it should create an install event if there's an existing meter on the SP
- or it should issue a Install Meter FA that will go to OFSC
and because of this, we are unable to start services
