Symptoms

Blockchain Ledger Summary data(sys) showing the chaincode committed by the user which doesn't exist.

The chaincode upgrade by some other user however, the "Initiator" field is shown as user who already left the organization long time back. Is the userID of the person who deployed the chaincode first time on the peer and channel shown as "Initiator"?
