Symptoms
Checklist Transactions Error every day in Prod.
This is for Process Name: Checklists Action Occurrence
Users are not experiencing any issue. They are getting checklist tasks allocated and getting notifications.
This is causing lot of confusion to the admins and requesting a way to stop these error transactions.
TAC (Transaction Console/details) Log shows below error:

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Tools > Transaction Console
2. You see that transactions with Process Name: Checklists Action Occurrence are having a status = red-x
3. Open the transaction and see TAC Diagnostic to see fault
