PPR payment failed formatting and pending action status

Customer is facing an error upon initiating a PAYMENT PROCESS REQUEST the status returned is: FAILED FORMATTING AND PENDING ACTION and upon checking the process logs, it saying it encounters an error in one of the backend plsql package below:
AP_PMT_CALLOUT_PKG.payments_completed

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. Initiate a PPR (Payables > Payments > Submit a PPR )
2. Approve the PPR until the end
3. Watch as the PPR fails


BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot transmit payment files
