Service Request Created From DCS Portal Displays Values From Previously Created Service Request (Doc ID 2898652.1)

Symptoms

ACTUAL BEHAVIOR
---------------
When two service requests are submitted from the same user in succession on the DCS portal the second request contains some values from the first service request.



EXPECTED BEHAVIOR
-----------------------
Expect that the SR will not be created with any values from the previous SR

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to DCS portal
2. Create a service request
3. The user creates a second service request and does not populate the address.
4. The address is populated from the previous SR.
