Submit Order Throws Error "The following items have not been completely assigned to shipping groups"

Symptoms
This error can often occur when merging cart occurs that enforce single shipping group, that is, items from guest cart are moved to user cart. This error can also occur when perform splitting shipping groups, that is, splitting the items from one shipping group to multiple shipping groups. For the latter case, please refer to "When implementing split shipping, "The following items have not been completely assigned to shipping groups" error when updating an order. Status 400. (Doc ID 2502749.1)".

STEPS
-----------------------
1. Login and add an item to cart.
2. Logut
3. Add the same item to guest cart.
4. Go to checkout and login during checkout.
5. When submit the order following error occurs:
